WHAT IS THE COMMUNITY PARTNERS PROGRAM?
The Community Partners Program (CPP) is part of the Australian
government Ageing and Aged Care Program. The purpose of the CPP is to fund
projects that will work with aged care services so that older people from
culturally and linguistically diverse communities gain access to, and equity in,
aged care services. The purpose of the Australia Government Ageing and Aged Care
Program is to enhance the quality of life for older people through support of
active, healthy ageing and the provision of appropriate high quality and cost
effective care services for frail older people.
THE AIMS OF THE PROJECT

To increase access of Maronite, Iranian, Afghan and Iraqi
frail older people into culturally appropriate aged care supports and
services

To support aged care services in Northern Sydney by
providing them with the relevant information that would assist them in the
delivery of services and programs to the target groups

To identify any emerging needs and issues of the Maronite,
Iranian, Afghan and Iraqi frail aged groups in Northern Sydney and to ensure
the formulation and adoption of strategies responsive to the needs/issues of
the target groups
